  • Scott Braunstein, MD

    Scott Braunstein, MD

    Dr. Braunstein has practiced in Southern California for 20+ years, and has become one of the most widely known and respected physicians in his field.  He trained in Internal Medicine at Cedars Sinai and Emergency Medicine at UCLA, and is double board certified in both specialties. Prior to joining Sollis, he was a Clinical Instructor of Emergency Medicine at Cedars-Sinai for 15 years, where he served on a number of leadership committees.  Dr. Braunsteindirected the annual Cedars Sinai Emergency Medicine Symposium, where he delivered dozens of cutting edge lectures, many of which were nationally published. Dr. Braunstein has also served as Airway Management Physician for the LA Rams of the NFL, and has provided expert commentary for CNN, ESPN, NBC, Forbes, and many other media outlets. 

  • Candace Dellacona

    Candace Dellacona is a principal and shareholder in Offit Kurman’s Estates and Trusts Practice Group. Candace is considered “a family’s lawyer,” however you may define family. She works closely with multiple generations of family members to protect what they have throughout every phase of their lives. Candace constructs wills, trusts, and medical directives that address each client’s unique goals, whether they are starting adult life or experiencing a significant event such as a change in marital status, welcoming a child, diagnosis of illness, or death of a loved one.

    Candace works with clients to proactively protect financial assets, arrange care, establish medical directives, mitigate the burden of estate taxes, and address business succession planning. Candace also navigates families through the probate process, ensuring that an estate is administered efficiently and properly.

  • J. Levi Chazen, MD

    J. Levi Chazen, MD is a Professor of Radiology at Weill Cornell Medicine and Director of Spine Imaging at Hospital for Special Surgery. Dr. Chazen’s clinical expertise is in Neuroradiology with a focus on brain and spine imaging as well as imaged-guided spine interventions. Dr. Chazen is past-president of the American Society of Spine Radiology (ASSR) and a senior editor at the American Journal of Neuroradiology (AJNR). He has over 87 peer-reviewed publications and his research interests focus on advanced brain and spine imaging, brain health imaging, and CSF leak diagnosis and treatment.

    Dr. Chazen joined the Advisory Team at Appian Care to support its mission of delivering expert, personalized guidance in brain health and aging—bringing cutting-edge academic innovations to a broad community of patients and families.
